An exploration of the barriers that influence the patient discharge experience: a multidisciplinary team perspective

This research explores the barriers that influence the discharge experience of patients from the viewpoint of the multidisciplinary team. A mix-methods study design was adopted underpinned by an Improvement Science Framework. The findings indicate that the multidisciplinary team are influenced by tradition when giving care to their patients.
