Training community health nurses and practice nurses – self-management support theory and practice – motivational interviewing, goal setting, action planning and problem solving skills – COPD specific clinical management • Aim to develop collaborative interpersonal relationship with focus on acquiring health behaviours and skills to achieve relevant goals in life – Use cycle of initiation and follow-up phone calls – Open ended questions and reflective listening – Offer “menu” of health-behaviours – ‘SNAPPS’: Smoking, Nutrition, Alcohol, Physical activity, Psychosocial well-being, Symptom management
